                           Biomechanical Aspects in Orthodontic Treatment

Knowledge of biomechanics in orthodontic treatment assists treatment, for both sliding and segmental arch techniques. Knowledge of arches made from new materials is necessary, particularly when treating adults different cases. This seminar will present the fundamentals of biomechanics illustrated by case-studies and scientific research.

New materials assist in the alignment and also simplify biomechanics. The new nickel-titanium (NiTi) arches and springs are customised for sliding mechanics. They will be presented with reference to case-studies and participants will be able to use them on simulated models. Orthodontic treatment of adults and difficult cases requires segmental arches. These can be used in combination with sliding mechanics. The lecturer will discuss closing spaces through controlled and uncontrolled tipping. Molar intrusion and uprighting feature are the aims of orthodontic treatment in adults and in first molars extraction cases. The lecturer will discuss biomechanics of different systems on treated patients and participants will be able to use them on simulator models.

                           ACTIVE AND PASSIVE SELF-LIGATION: CLARIFICATION FROM CONTROVERSY

In professional circles there is still an on going on a debate about which of the self-ligating techniques has more advantages: the active or the passive one. To clarify this dilemma we have asked two highly competent professionals, Dr Alan Bagden and Dr Friedrich Byloff, to present their views. They will explain the characteristics of two very popular systems, Damon and Speed.

In conclusion both lecturers will be invited to participate in a panel discussion, led by the well known expert in the field of biomechanics, Professor Hans-Peter Bantleon. We hope that in this way the attendants to the course will be well acquainted with the real advantages of the presented selfligating systems.

Professor Hans-Peter Bantleon is an expert in the field of biomechanics in orthodontics. The contemporary and biomechanical aspects of the two different self-ligating systems (active and passive) will be highlightened at a panel table discussion on the latest clinical and sicentific reseach results.

                           TIME: THE FOURTH DIMENSION IN ORTHODONTICS AND DENTOFACIAL ORTHOPAEDICS

The issue of the ideal time when to start orthodontic treatment is vital in the growing patient. In many instances treatment timing may have a greater impact on treatment outcomes than the specific appliance used or its clinical management.

This course will describe the role of treatment timing in different types of dentoskeletal disharmonies: Class II and Class III malocclusions, transverse discrepancies, vertical problems (both deep and open bite malocclusions). The use of biological indicators of individual skeletal maturity, with special regard to the cervical vertebral maturation method, will be explained in order to define the optimal timing of orthodontic/orthopaedic intervention.

A series of long-term retrospective and prospective clinical trials will highlight the effects of treatment timing in orthodontics and dentofacial orthopaedics.

                           THE FACIAL PROFILE – CHANGES WITH AND WITHOUT TREATMENT

Urban Hägg, Hong Kong

Dr. Hägg received his orthodontic and research training at the Faculty of Odontology, University of Lund, Sweden (DDS, Odont Dr). In 1989 he joined the University of Hong Kong (FHKAM, Dental Surgery and CDSHK, Orthodontics), where he was the Postgraduate Director in Orthodontics until 2006. In 1993 he became Chair Professor, and in 2004 Associate Dean of Postgraduate and Continuing Education. He is Honorary/Visiting Professor at the five dental schools in China. He has served as President of Hong Kong Society of Orthodontists, President of the College of the Dental Surgeons of Hong Kong, and in 2008 he became vice president of the Asian Pacific Orthodontic Society. He holds an honorary FDS of the Royal College of Surgeons of Edinburgh and is a member of the Angle Society (North Atlantic Component).

His thesis on ‘The pubertal growth spurt and maturity indicators’ (1980) was followed by publications on growth and development, and outcome of orthodontic treatment related to maturity. His other fields of his interests are clinical and experimental growth modification, and quality of life related to malocclusion/orthodontic treatment.

10:00                   KL 01 ETHICAL ISSUES IN ‘ENHANCEMENT’ MEDICINE

Jože Trontelj, Slovenia

Dr Trontelj, Medical Doctor, Professor of Neurology, University of Ljubljana, has authored, co-authored, more than 150 articles in journals, over 30 book chapters and two books, mainly on electromyography and the physiological basis of neurological disorders. He has also published over 90 publications on bioethical issues. He has been a member of the Slovenian Academy of Sciences and Arts since 1991 and President of the Academy since 2008. In 1995 he was appointed Chairman of the National Medical Ethics Committee of Slovenia. He is the Slovenian delegate to the Steering Committee on Bioethics (CDBI) of the Council of Europe and as member of the Working Party that drafted the Additional Protocol to the Convention on Human Rights and Biomedicine, on biomedical research.

11:00                   KL 02 BIOTECHNOLOGY AND THE FUTURE IN ORTHODONTICS

Fraser McDonald, UK

Professor McDonald obtained his undergraduate qualification in 1980 at Birmingham University. After three years of general dental training he joined the orthodontic training programme at the Royal Dental School, London under the supervision of Professor William Houston. He became a lecturer in oral biology integrating physiology and orthodontics at the University of London. He subsequently obtained qualifications in specialisation and completed his PhD in 1992. He was appointed as professor in 1999 and was awarded the James IV professorship of the Royal College of Surgeons of Edinburgh in 2002. He is currently Head of the Department of Orthodontics at the Dental Institute, Kings College London.

He has published over 75 full papers, 150 abstracts, three books and supervised over 90 Masters’ reports. Since 1993 he has been editor of the European Journal of Orthodontics. His current research interests include cleidocranial dysplasia and signalling mechanisms in osteoblasts.

In 1997 the European Orthodontic Society held the first examination for the European Board of Orthodontists (EBO). The purpose of the EBO from the beginning was to enhance the standards of orthodontic treatment throughout Europe by providing a standard against which the orthodontists who so desire can be judged independently of national examinations by an expert panel of European orthodontists nominated by the Council of the European Orthodontic Society. Nearly 100 orthodontists are now board certified and it is hoped that many more will follow!

The EBO is a Board of Excellence. Preparing for the examination gives the opportunity to scrutinize the quality of one's clinical work. The successful candidate shows that his/her knowledge and the treatment results are of the highest level.

What advantage does it have to become a member of the Board? There is no material gain, nor are there any other privileges. There is, however, great personal satisfaction if one's peers acknowledge
one's level of knowledge and clinical skills. The process of preparation and sitting the examination  is an invaluable learning experience and will improve the quality of clinical treatment from which future patients may benefit. In return, this may well have a positive effect on the long-term economic situation of the orthodontic practice.

Professor Wichelhaus obtained her Habilitation, Phd, Ulm University in 1996. From 1990 to 1999 she was Assistant Professor and alternate Director of the Division for Orthodontics, Ulm University. In 1996 she was a Research Fellow at the Dept. of Orthodontics, Harvard University, Boston USA. She was Director and Chairperson, Dept. of Orthodontics and Pediatric Dentistry, University of Basel, Switzerland from 1999 to 2008 and since 2008 has been Director and Chairperson, Dept. of Orthodontics, Ludwig-Maximilians University Medical Center, Munich, Germany. In 1995 she received the Walter-Engel-Prize for recognition of scientific developments in orthodontics and the prize for the best annual publication of the German Society for Orthodontics. Professor Wichelhaus’ main areas of interest are: Development and testing of the spring systems and wires for fixed appliances; Biomechanics of the oro-facial system: in vivo and in vitro studies on tooth movement; Mechanobiology; Biocompatibility testing of orthodontic materials; Radiology of bone structures; Genetics and Influence of the muscular system on functional processes.

16:30                    KL 04 THE USE OF BIOMECHANICAL MODELLING
                                        TO PREDICT SURGICAL CHANGE

Stephen Richmond, UK

Professor Richmond was born in Liverpool. He gained his dental degree in Sheffield in 1979 followed by an orthodontic specialist training qualification in Cardiff in 1984. He was a Lecturer and Senior Lecturer at the University of Manchester from 1985 to 1993 and attained his doctorate in relation to quantifying orthodontic treatment need and outcome (standards) in the General Dental Services in England and Wales. He returned to Cardiff in 1993 and is currently Head of Applied Clinical Research and Public Health. He is best known for developing and implementing the use of the PAR, IOTN and ICON indices and assessing the cost-effectiveness of orthodontic treatment. His recent studies have focused on three-dimensional facial growth and growth prediction (12-17 year olds) in different population groups and the influence of environment and genetics on facial shape. Recent research grants have exceeded € 3 million supported by over 200 publications.

08:30                   KL 05 SELF-LIGATING BRACKETS: CUI BONO?

Frank Weiland, Austria

After studying dentistry at the University of Amsterdam, Dr Weiland received his postgraduate orthodontic education in Germany and Austria. He holds a Dr.med.dent. from the University of Bern (Switzerland) and a PhD from the University of Graz (Austria). Among his special areas of interest are biomechanics, long-term results of orthodontic treatment, time-efficient orthodontic treatment and external apical root resorption.

He is in private practice and is an associate professor at the University of Vienna. He is a member of the editorial board of several international journals. He is a member of the European Board of Orthodontists (EBO) and the Angle Society of Europe, and in 2009 was appointed President of the EBO. He is a recipient of the Beni Solow Award for the best article published in the European Journal of Orthodontics in 2003.

Alan Bagden, USA

Dr. Bagden undertook his dental training at the University of Pennyslvania (1978) and his orthodontic residency at the University of Maryland (1987). He has been in the daily practice of passive self-ligating orthodontics in Springfield, VA, USA, since 1999, and has been actively involved in the development and education of passive self-ligation since the inception of the Damon System. He has successfully treated more than 2000 Damon cases. Dr Bagden received certification by the American Board of Orthodontics in 1992. He has co-authored The Damon Workbook, hosted numerous in-office seminars and has presented lectures on techniques of mechanics to the American Association of Orthodontists the European Orthodontic Society and Canadian Orthodontic Association meetings. He has given more then 150 seminars in more than 35 countries.

11:00                   KL 07 UPRIGHTING MISSCONCEPTIONS ABOUT SELF-LIGATION

Friedrich K. Byloff, Austria

Dr. Byloff (M.D., D.D.S., DIP. Orthodont) entered the health-care profession in Graz, Austria, where he studied general medicine from 1976 to 1982. He then served a one year internship in South Africa. Upon his return to Austria, he began the study of general dentistry. After completion of his general studies in 1985, Dr Byloff worked for an additional year in the Department of Maxillo-facial surgery. He then entered and completed his speciality in orthodontics at the University of Geneva Switzerland (1986–1989), where he subsequently stayed another two years as an instructor (1989 – 1991). He obtained his PhD (Privatdozent) at the University of Graz in 2006.

Dr Byloff continues to provide assistance to the University of Geneva orthodontic programme as an instructor and to the Orthodontic Department, University of Sydney, as a visiting instructor. Dr Byloff is on the editorial board of the American Journal of Orthodontics, the Journal of Orofacial Orthopedics. He is a full member of the European Angle Society.

Dr Baccetti graduated in dentistry and obtained a PhD in Dental Sciences at the University of Florence, Italy. He is currently Research Professor at the Department of Orthodontics of the same University, and ‘TM Graber Visiting Scholar’ at the University of Michigan, USA.

He has published over 150 articles in peer-reviewed journals, is a member of the Editorial Board of the European Journal of Orthodontics, and the recipient of several international scientific awards. He has lectured in over 30 different countries. Dr Baccetti’s expertise focuses on dentofacial orthopaedics, growth modification, and treatment timing.

Professor Ihan Hren graduated from Ljubljana Faculty of Medicine in 1987. She obtained her Master of Science Degree in 1991, her Ph.D. degree in maxillofacial and oral surgery in 1999. She was appointed to the Speciality Board of maxillofacial and oral surgery in 1985 and was made assistant professor of maxillofacial and oral surgery in 2000. Since 2003 she has been head of the faculty department of maxillofacial and oral surgery, Medical Faculty, University of Ljubljana.

Moriyon Uria received his medical degree M. D. in 1975 at the University of Navarra, Spain. Two years later, in 1977, he got his D. D. S. (Doctor of Dental Science) at the University of Madrid. After that he undertook training in Functional Orthopedics and Orthodontics with Dr. Moriyon Costales in Madrid. From 1986 to 1988 he worked with Dr. Roth and Dr. Williams in the Centre for Functional Occlusion at postgraduate studies. He finished his specialization in orthodontics at the University of Oviedo in 1996. Dr M. Ljubljana, the Capital of Slovenia, is a dynamic city located in the centre of the country. Ljubljana’s historic centre has managed to retain traces from all periods of its rich history; from the legacy of Roman Emona, through to the Renaissance, Baroque and Art Nouveau periods characterized in the house fronts and ornate doorways of the city centre, the romantic bridges adorning the Ljubljanica river, the lopsided rooftops and a park reaching deep into the city centre. Our tour will include visits to the Town Hall, St Nicholas Cathedral and the open air Central Market, which sells fresh local produce and arts and crafts.

We will then take the funicular to the medieval castle of Ljubljana situated at the summit of the hill that dominates the city. The present castle was built between 1485 and 1495 with the purpose of defending the empire against an Ottoman invasion as well as a peasants’ revolt. The Silver Mill Competition will take place during the visit to Ljubljana.

Chung H. Kau is Professor and Chairman at the Department of Orthodontics, University of Alabama. Previously he was Associate Professor and Director of the 3D Imaging Facility at the University Of Texas Health Science Center. He is as an active clinical practioner and researcher with a keen interestin three-dimensional and translational research. His research facility is in possession of the 3D Sirona Galileos cone beam scanner and the 3dMD surface imaging device.
He also has a number of international collaborations in the field orthodontics. These include the understanding facial morphology in populations and facial asymmetries in hip dysplasia children. His work has brought him to understand the population based differences in Slovenia, Finland, Wales, Hungary and Egypt. He has formed good partnerships and research relationships in these countries. He has served as a research mentor to a number of researchers, residents and pre-doctoral students. Chung H. Kau is a well sought after and invited speaker on his clinical research and has shared his work on the international stage that includes Australia, New Zealand, North America, Western and Central Europe, the Baltic States, Hungary and the Far East. He actively contributes andpublishes in the orthodontic literature and currently has over 100 peer-reviewed publications, conference papers and lectures.
